What formulas does it use?

The box multiplies length times width times height; the cylinder uses pi times radius squared times height; the sphere uses 4/3 times pi times radius cubed.

What is the most common volume mistake?

Mixing units, such as entering length in metres and height in centimetres. The result inherits the cube of whatever unit you enter, so centimetre inputs give cubic centimetres - measure every side in the same unit first.
